Need advice on pouring a new concrete pad for a FL front load washer and dryer. The washer rests on a concrete slab in basement, close to a floor drain. The problem is the floor slopes about two inches from the back of the washer to the front. With the front legs fully extended, it's not level front to back.

I'm thinking about a 4" pad, 60 inches wide. Would it make sense to leave the word forms in place to act as a lip to prevent the washer, if it ever walked again, from walking off the pad? I'd hate for it to walk off the pad and fall.

when I poured a similar slab in my garage for my FL. I set 1/4"X2" rubber washers where the feet go. After the concrete set, I pulled out the washers and there are 4 perfect feet holders for the washer and it can't walk... of course now that it's perfectly level it's never moved. I probably wouldn't do it again as it's a PITA to move the washer if needed. Besides, even if it does walk, 3-4 inch drop won't be enough to tip the washer over.
